Freshmen :
  • Senior High School Report Card (original)
  • MSU SASE / CET Rating Card (original)
  • Certificate of Moral Character (original)
  • Birth Certificate / SECPA (photocopy)
  • 1 Long Brown Envelope
  • Police Clearance (original)
  • ID Picture (3 pcs 2x2) with White Background and Name Tag

Transferee and 2nd Degree :
  • TOR (Evaluation copy)
  • MSU SASE / CET Rating Card (for transferee)
  • Honorable Dismissal
  • Certificate of Moral Character
  • Police Clearance
  • Birth Certificate / SECPA (PSA) (Xerox Copy)
  • ID Picture (3 pcs 2x2) with White Background and Name Tag

Process

Receive the enrolment form Advice to go to his/her respective college to get approval from his/her Adviser and Dean. None Acting Director Enrollment Form
Present the enrolment form for approval for the Dean and Adviser Approve the student by the Dean and Adviser None Adviser and Dean Enrollment Form
Receive the approved enrolment form. Advise the student to return the admission office for ID Number. Adviser and Dean Enrollment Form
3 Present the enrollment form that has been approved by the Adviser and Dean. Entry of vital information into the Campus Registration Database and secure University ID number. None Staff University ID number
Advise the student to return to the respective college to present the ID number None Staff University ID number
4 Return to the respective college to present the ID number Provide the Parliamentary Registration Form (PRF) None Assessor PRF
5 Visit the individual offices:
