I. AMPHITHEATER

General Rules

1. A completed form application must be submitted to the OVCAA. Such an application should be submitted at least sixty (10) days prior to the event.

2. A contact/responsible person must be named on the application who will oversee the event.

3. No outside food or drink is permitted. Alcohol is not permitted.

4. If any damages occur to the venue, the person accountable must be responsible for the cost of repairs.

Amphitheater Closing Procedure

1. The seating area, stage, backstage and downstairs are to be cleared of all trash.

2. Tables and chairs must be put back in their original area.

3. All trash is to be placed in the venue Dumpster.

Guidelines

1. No rental fee for use of the Amphitheater, if the event is classroom or school related activities.

2. Must follow Amphitheater closing procedure above.

3. The Administration will designate a facility manager to be responsible for opening and closing the facility.

4. Amphitheater personnel will be responsible for opening and closing the facility.

5. Providing stage lighting and sound equipment is the responsibility of the lessee.

6. Any alterations to the facility, including for lights, sound and decoration, must have the prior approval of the Amphitheater personnel.

Guidelines

1. Cleanliness should be observed before, during, and after each laboratory period.

2. Eating is strictly prohibited inside the laboratory.

3. The only thing that should be in the cubicle/workstation is the headset. Any wet liquid or food is not allowed in the area.

4. Do not overload electric circuits.

5. Do not stay inside the laboratory during your vacant hours.

6. Students’ backpacks are required to be stowed below their chairs.

7. Do not sit on lab tables and cubicles.

8. The used chair(s) should be tucked under the table or placed in its proper place.

9. To ensure the quality of the headphones and response keys, you need to handle them with care and avoid pulling on the wires.

10. Place the headphone on the rightmost part of the cubicle when not in use.

11. There is a prohibition against tapping other cables for use on each TV and projector because this practice can result in port distortion.

12. The instructor should unplug the control panel when not in use and before leaving the speech laboratory.

15. Report immediately to the laboratory custodian any untoward incident, water leakage, faulty wiring, and other hazards.

III. MSU BUUG GYMNASIUM

By adhering to these rules and regulations, organizers can ensure a smooth and successful event while maintaining a safe and respectful environment for all attendees.

1. The organizer of the activity must complete a duly approved and signed form for Academic/Extra-Curricular Activities from the OSAS or an approved special order from the office of the chancellor. And complete the Facility Usage Form.

2. Reservations are on a first-come, first served basis and can only be confirmed upon receipt of the necessary forms or documents.

3. The gymnasium must be used only for the purpose stated in the reservation request

4. The organizers are responsible for the setup and arrangement of the hall. All arrangements must comply with safety regulations and fire codes.

5. Events must commence and conclude at the specified times in the reservation request.

6. Organizers are responsible for the security of their participants and their belongings.

7. The gymnasium must not be overcrowded, and the number of attendees must comply with the gym’s maximum capacity.

8. Fire exits and aisles must be always kept clear.9. Smoking, consumption of alcoholic beverages, open flames, and hazardous materials are strictly prohibited inside the gymnasium.

10. The gymnasium must be returned in the same condition it was provided.

11. Organizers are responsible for cleaning up the gymnasium after the event.

12. The volume of audio-visual equipment and speakers must be kept at a reasonable level to avoid disturbing neighboring areas or events, particularly during nighttime.

13. The office of the Sports Development and Physical Education is not responsible for any loss, damage, or theft of property belonging to the organizers or attendees.

14. The organizers are liable for any damage caused to the gymnasium premises or equipment during the event.

15. Students are not allowed to loiter/roam around inside the gym during physical education classes.

IV. RESEARCH HALL/ RESEARCH CONFERENCE ROOM

1. Ask for the Facility Usage Form from the OVCRE.

2. Fill out the needed information (type of event, no. of participants, date of event, facility to be used) and accomplish all the signatories in the form.

3. Submit a copy of the Facility Usage Form to the OVCRE and a valid ID for confirmation of the reservation.

4. Strictly observe “Clean as You GO” (CLAYGO). Those who will violate this will be subjected to a penalty of 200.00-500.00 pesos.

Note: The Research Conference Hall is strictly for research and extension activities only and other faculty seminars with a limited capacity of 60 persons. Unauthorized use of the said facility will result in a penalty of 500/hour. In case of delayed return of facilities such as Projector, HDMI, extension wire, a penalty of 300.00/ day
